The T1D MOD Squad Educational Scholarship Program is an opportunity for high school seniors who successfully manage the challenges of a life with T1D. Four (4) $500 scholarships will be awarded to be used toward higher educational tuition, including junior colleges or trade school. We will conduct a confidential, holistic review of the information you include in this application to select four qualified candidates. Community engagement, academics, and essay responses will constitute the basis of selection. Please complete the following application including your personal information and written essays.
All high school seniors (US citizens/residents) whose parent or guardian has been a member of T1D MOD Squad for at least one (1) year are strongly encouraged to apply. A thorough review of each application will take place, and the selected candidates will be notified in early June 2023 via telephone. Applicants who are not selected will be notified during the same time frame in writing via email. Selected applicants will also be featured on our website.
